Since 1995, Community Medical Clinic has been providing quality care to the people who reside within Pana Community Hospital's service area, which spreads across four counties.  Our dedicated team of Family Medicine Physicians, Family Nurse Practitioners, and professional staff offer care in three locations to serve you better — Pana, Assumption and Nokomis. The Pana office offers evening hours three days a week to accommodate our patients busy schedules.  Call our office at 217-562-2544

Medicare Well Visits

A Medicare well visit is an opportunity for patients to sit down to look your care as a whole. We make sure your chart is up to date and make a plan for screenings and preventative care for the coming year. Medicare participants do not pay for this yearly visit.


A wellness visit will include the following:

  • Review of medical and family history
  • Review of current medications and other providers care
  • Check height, weight, and blood pressure
  • Review preventative and screening services to get you up to date (vaccinations, colonoscopy, mammogram, bone density, etc.)

Medications are not prescribed at these visits, which must be obtained at regularly scheduled appointments.

Chronic Care Management

The purpose of a Chronic Care Management program is to customize and coordinate the healthcare of an individual to their specific needs.  The program ensures you will receive the right care, in the right form, from the right provider, and at the right time to care for your chronic condition . Examples for chronic conditions may include but are not limited to the following:

 

  • Alzheimer's disease and related dementia
  • Arthritis (osteoarthritis and rheumatoid)
  • Asthma
  • Atrial fibrillation
  • Autism spectrum disorders
  • Cancer
  • Cardiovascular Disease
  • Chronic Obstrcutive Pulmonary Disease
  • Depression
  • Diabetes
  • Hypertension
  • Infectiuous diseases such as HIV/AIDS

 

Should an individual choose to participate in the program, you will be assigned to a care manager who will work with you to better manager your healthcare.  Participation is voluntary and individuals may opt out at any time.  By participating individuals will receive:

 

  • Personalized healthcare plan to assist you in reaching your healthcare goals
  • Monthly calls with a care manager to review/update your care plan and address any of your helathcare needs
  • Regularly scheduled preventive services and medication monitoring if needed
  • 24/7 access to a member of our staff for any urgent chronic care needs
  • Have the opportunity to communicate with the care team by telephone, secure messaging, and email

Insurance & Information

Insurance

We accept all major insurance carriers, Medicare, and Medicaid.  Please check with your individual insurance provider to verify coverage.

Prescription Medications

Medications should be discussed and refilled during an appointment or requested online through the secure patient portal.  Please allow 24 hours for all refills.

 

Appointments

To schedule an appointment, call any of our three office locations or request an appointment via the Patient Portal.  If you are unable to keep a scheduled appointment, please call our office to cancel or reschedule. 

 

Emergency Care

In an emergency situation, patients are asked to got to the nearest Emergency Department.  

**After hours, please call 217-562-2544 to receive instructions for urgent and emergent care needs.

 

Walk-In Clinic
If our offices are closed and you have a non-emergent need, patients may go to ExpressCARE located at Pana Community Hospital.  Use the Emergency/ExpressCARE entrance to be seen.  The hours for this convenient service are Monday-Friday-7:00 am - 7:00 pm and Saturday and Sundays and holidays 10:00 am - 4:00 pm. ExpressCARE is open all holidays except Thanksgiving and Christmas. No appointments are needed!!

Community Medical Clinic offers patients access to their medical information securely through an online patient portal. With the HealthELife online patient portal, you can conveniently and securely manage your healthcare needs on your desktop computer or through a free mobile app.

You can use your HealthELife portal to:

  • Send secure messages to your providers
  • Request prescription refills
  • Request or cancel appointments
  • Request and Start a Video Visit 
  • View lab, radiology  results
  • Look up your vital signs, medications, immunization history, and much more
  • Receive reminders for upcoming visits 
  • Pay your bill online 

All you need to activate this service is a valid e-mail address for each person registering for the portal.  Just provide your email address to our registration staff to get the process started.
