Who Qualifies for Hospice?

To be eligible for Quad County Hospice, a patient must:

  • Have a terminal illness diagnosis with a life expectancy of six months or less (as certified by a physician).
  • Be receiving palliative care only (focused on symptom relief rather than curative treatment).
  • Have a responsible primary caregiver available to assist with care.
  • Understand their prognosis and choose hospice care as their preferred option.
  • Reside within our service area (Christian, Montgomery, Shelby, Fayette, and Macoupin Counties).
  • Have an attending physician who will oversee hospice care.
  • Be accepted without regard to race, religion, national origin, disability, diagnosis, lifestyle, or ability to pay.

Administrative Volunteers:

Prefer to help behind the scenes? Administrative volunteers play a key role in supporting our hospice operations. Tasks may include:

  • Making “Care Calls” to check in on patients and families

  • Filing and data entry

  • Preparing admission packets and mailings

  • Writing “Thinking of You” and Thank You cards

  • Assisting with general office work

Patient & Family Volunteers

Our Patient & Family Volunteers provide companionship, emotional support, and respite care for hospice patients and their loved ones. Visits take place in private homes, assisted living facilities, nursing homes, and hospital inpatient rooms. Ways You Can Help:

  • Friendly neighbor visits to reduce loneliness

Each November, Quad County Hospice holds an Annual Memorial Service to honor those who have passed. Volunteers help with:

Referrals

Anyone can make a referral to Quad County Home Health, Hospice, or Medical Equipment services—whether you’re a physician, hospital, social worker, discharge planner, family member, friend, or the patient themselves. For home health, a skilled nurse will provide a free in-home assessment and work with the patient’s physician to develop a customized care plan. For hospice, our Patient Care Coordinator will review the referral, consult with the attending physician, and create a personalized care plan to meet the patient’s needs. For medical equipment, no physician referral is required for non-prescription items, making it easy for patients, families, and caregivers to get started.
