which has been serving patients since 1984.  

 

Quad County provides durable medical equipment, home health services, and hospice programs to Christian County, Shelby, Montgomery, and Fayette Counties.  
Now our Home Health and Hospice service covers Macoupin County!

Quad County Home Health was established in 1984 to meet the home health needs of residents of Pana and surrounding communities.  Our professional staff provides intermittent home visits which enable our patients to continue to live at home.  Our purpose is to promote, maintain, and restore health.  Through our home visits, we are able to maximize an individual's level of independence while minimizing the effects of disability or illness.

Occupational Therapy–Rebuilding skills to accomplish routine tasks

Home IV Services
Patients requiring intravenous therapy can remain at home and receive training and supervision of therapy by a licensed registered nurse.

 

Equipment and Supplies

Our agency will be glad to help you determine the types of equipment and supplies that will best suit your needs.  We can provide you with information on products such as wheelchairs, beds, oxygen, or emergency call systems.  Upon delivery of equipment, staff will train you and your family members on correct usage.  A 24-hour call service is available to answer any questions that may arise.

 

Payment

Payment for Quad County Home Health services may come from one source or a combination of sources including private payment, commercial insurance, Medicare and Medicaid.  

 

Referrals

Anyone can make a referral to Quad County Home Health.  A skilled nurse will provide a free assessment visit to determine the patient's specific needs, and will then discuss the findings with the patient's physician.  

 

Hours of Service

The Quad County Home Health office is open Monday-Friday from 8:00 am-4:30 pm.  Our staff will be happy to answer any quesitons or arrange for services to begin.  A nurse is available 24 hours a day, 7 days a week, even on holidays.  If you call when the Quad County Home Health office is not open, your call will be answered by the on-call nurse.

Quad County Hospice was established to assist patients facing terminal illness in such a way that they are able to live in the comfort of their own homes.  Family members are encouraged to be a part of the patient's care team in support of their loved one.  They receive supportive
ser
vices throughout the patient's terminal illness and follow-up bereavement service.  

 

Hospice emphasizes controlling pain and managing symptoms for patients, not curing disease.  We specialize in supportive care for both the patient and the family during what can be a very difficult time.  The goal is to enhance the quality of life remaining when a disease is irreversible and life expectancy is limited.

 

Admission Criteria

A Quad County Hospice patient:

  • has a diagnosis of terminal illness with a medical prognosis of six months or less.
  • must be receiving palliative care only (management of symptoms rather than treatment of the disease).
  • has a competent and responsible primary care person available.
  • is aware of the prognosis and treatment options and chooses hospice care.
  • lives within the service are, which includes Christian, Montgomery, Shelby, Fayette and Macoupin Counties.
  • has an attending physician who will medically direct the hospice care.
  • is accepted without regard to race, religion, national origin, handicap, diagnosis, lifestyle difference, or ability to pay.

Quad County Hospice can provide durable medical equipment, medical and surgical supplies, and medications related to terminal disease process. 

 

Services

The Hospice care team is a group of very dedicated professionals.  They possess a wide variety of skill and provide care a sdirected by the physician.  Their expertise includes:  Physician service, Nursing services, Social Services, Dietary services, Volunteer services, Physical, occupational, and speech therapy, bath aide services and pastoral/bereavement care.

 

Payment

Payment for Hospice may come from one source or a combination of sources including private payment, private insurance, Medicare hospice benefit and Medicaid.

Anyone can make a referral to Quad County Hospice.  The Patient Care Coordinator will discuss referrals with the attending physician.

Emergency Response System

CareLink is an emergency response system available through Quad County.  With the touch of a button, the patient is able to set into motion the predetermined plan for getting help to the home immediately.  This service provides peace of mind for both the patient and family.

 

 

 

 

Supportive Services

To ensure the maximum benefit from our equipment placement, we provide the following supportive services for each customer:

  • Individualized training, written instructions, and easy to understand customer information.
  • Free delivery and set-up of all equipment, usually the same day we receive the physician approval.
  • Continued contact with our physician and other healthcare providers to ensure that your equipment continues to meet your changing healthcare needs.

 

Referrals

Anyone can make a referral to Quad County Home Medical Supplies & Equipment, Inc.  Many times referrals come from social workers, hospital discharge planners, physicians, nursing associations, patients, family members, or friends.  There is no physician referral needed for non-prescription items. 

 

Payment

Quad County Home Medical Supplies & Equipment, Inc. assumes the responsibility for billing insurance companies and Medicare for equipment and services.  This relieves the patient and family of the burden of frustrating paperwork.

Quad County Hospice Volunteers are a very special kind of volunteer.
Their hearts and their unconditional kindness cannot be measured.

 

 

Quad County Hospice Volunteer Program has many types of volunteering opportunities.

Administrative Volunteer

Volunteers who prefer to work in an office setting can offer many clerical opportunities: Making “Care Calls” to Patients/Families, filing, computer work, help with mailings, writing Thinking of You cards and Thank You cards, making admission packets, and other general office assistance.

Patient/Family Volunteer

Quad County Hospice Volunteers provide companionship to patients/families which will enhance the quality of life.  Visits are done in Private Homes, Assisted Living Faculties, Nursing Home Care Faculties, and in an Inpatient Hospital Room. Volunteers are always needed for patient/family support. Volunteer support might include: short friendly neighbor visits, scheduled visits for family respite, providing companionship, writing letters for patient, reading to the patient for example: Bible, newspaper, mail, etc., watching TV, playing cards/games, looking at family albums, the options are endless.  

Veteran to Veteran Volunteer

Veteran Volunteers are placed with a Veteran Hospice Patient to provide a special bond of companionship. Veterans can talk about life events that they have experienced firsthand with a volunteer that will understand and relate to the Veterans mental needs.

Chaplain Volunteers

Spiritual support is provided by the Chaplain weekly or as requested to the hospice patient and or family/friends.

Other Volunteer Opportunities

  • Memorial Volunteers—Annual Hospice Memorial is in November. A Memorial Service is provided for families who have lost loved ones over the prior year.  Support from volunteers is needed with addressing invitations, decorating for event, serving refreshments, and socializing with bereaved families.   
  • Care Blankets—making handmade tie blankets with matching pillows that are given out to every hospice patient.
